The prescription plan has a formulary (preferred list of drugs) list. If the drug is on this plan and it is a name brand drug it will be $25.00, if it is a generic then it will be $10.00. If the drug is not on the formulary it will cost $50.00 (regardless of whether it is a generic or name brand drug). There is also a mail order plan that allows you to order 3 month's supply for 2 1/2 co-pays.
To see if your prescription is on this list, go to http://www.bcbsok.com/drug_formularies.html. You will click on "Formulary List" to see what drugs are on the preferred list. On the next page you can download the formulary list as a pdf file, or search for a drug by name.
You can print off this list to take to the Dr. Office visit to allow you Physician to see which drugs are in each tier.
Tier 1 drugs are $10.00 maximum
Tier 2 drugs are $25.00 maximum
Tier 3 drugs are $50.00 maximum
